Description
Arising near the origin of the anterior interventricular artery (left anterior descending, LAD), the conal branch courses anteriorly and superiorly across the right ventricular outflow tract (conus arteriosus) toward the pulmonary trunk. From an anterior external perspective, the LAD descends in the anterior interventricular sulcus between the right and left ventricles, while the conal vessel sits more superior and rightward, skirting the infundibulum just inferior to the aortic root. The ascending aorta and proximal pulmonary trunk anchor the superior field, with superficial epicardial coronary veins tracing alongside the arterial network over the myocardium beneath a thin pericardial covering. This conal branch matters because it supplies the right ventricular infundibulum and can form the clinically recognized arterial circle of Vieussens when it anastomoses with a conal branch from the right coronary artery. That collateral pathway becomes visible in LAD or right coronary stenosis and can affect ischemia patterns in the anterior right ventricle. Small vessel. Big implications for angiographic interpretation and for procedures near the right ventricular outflow tract. Cardiology and cardiothoracic surgery teams use this view when correlating coronary angiography or CT coronary angiograms with surface anatomy, since the conal branch can be mistaken for an early diagonal branch if you lose the landmark of the pulmonary trunk and infundibulum. It also fits well in teaching blocks on coronary circulation, epicardial versus intramyocardial vessel courses, and operative orientation during median sternotomy or RVOT interventions such as repair of tetralogy of Fallot.
