Description
Prominently on the anterior (sternocostal) surface of the heart, the anterior interventricular artery (left anterior descending, LAD) descends in the anterior interventricular sulcus toward the apex, accompanied by the great cardiac vein. A diagonal branch leaves the LAD obliquely, coursing superolaterally across the anterior wall of the left ventricle toward the obtuse margin, where it distributes to the anterolateral myocardium. Superiorly, the ascending aorta and proximal great vessels frame the coronary origins, while the pericardial covering is implied by the clean external contours and epicardial fat planes. Arteriolar branches spread over the ventricular surface. Short and direct. Clinically, the diagonal branches matter because they supply a sizable portion of the left ventricular anterolateral wall, and isolated diagonal occlusion can produce ST elevation in I, aVL, and the anterior precordial leads with focal wall-motion abnormality on echocardiography. Interventionalists often describe lesions by their relationship to the first and second diagonal branches (D1, D2) during coronary angiography, since these side branches influence stent strategy, risk of side-branch compromise, and interpretation of a proximal LAD stenosis. For surgical planning, diagonal targets are common in coronary artery bypass grafting, and their epicardial course helps orient the surgeon when the LAD itself is embedded in myocardium (myocardial bridging). Cardiology lecturers and anatomy faculty can use this anterior view to teach coronary surface anatomy, the LAD-diagonal branching pattern, and venous companions on the anterior heart, while medical publishers will find it suitable for chapters on ischemic heart disease, PCI, and CABG graft target selection.
