Description

Arising from the left ventricle, the aortic root (radix aortae) is seen from a posterior perspective as it continues superiorly into the ascending aorta (pars ascendens aortae). At its base, the aortic annulus surrounds the aortic valve, while the right and left coronary arteries originate from the corresponding aortic sinuses and course inferolaterally toward the atrioventricular and interventricular grooves. Posteriorly, the root sits anterior to the left atrium, with the pulmonary trunk positioned anterolateral and the superior vena cava more to the right. Orientation matters at the root because this is where valve pathology and coronary perfusion intersect. A posterior vantage helps clarify the relationship of the left coronary ostium to the left atrium and pulmonary artery, a relationship that becomes relevant during aortic root replacement, valve-sparing procedures, and transcatheter aortic valve implantation where coronary obstruction and ostial height are real procedural constraints.
