Description
Arising from the left coronary artery, the anterior interventricular artery (left anterior descending, LAD) courses inferiorly within the anterior interventricular sulcus toward the apex, running between the right and left ventricles on the sternocostal surface of the heart. Diagonal branches sweep laterally across the anterior wall of the left ventricle, while septal perforators dive posteriorly into the interventricular septum. Superiorly, the aortic root and proximal coronary origins sit at the base of the heart, and the pulmonary trunk lies anterior and leftward relative to the ascending aorta; accompanying venous channels, including the great cardiac vein, commonly parallel the LAD in the same groove. Coronary arterioles and surface vessels trace the epicardium, emphasizing the vascular map over the ventricular myocardium. This anterior perspective matters because the LAD supplies a large territory: the anterior left ventricular wall, the apex, and much of the anterior two thirds of the interventricular septum, including the bundle branches. Occlusion proximal to the first septal perforator or major diagonal branch drives the classic anterior ST elevation myocardial infarction pattern and can produce heart block, ventricular arrhythmias, and acute pump failure. For interventional planning, the LAD’s relationship to the interventricular groove and to the great cardiac vein is a practical landmark when selecting stent length, anticipating bifurcation disease at diagonal takeoffs, or describing lesions using the proximal, mid, and distal LAD segments. Use this rendering for gross anatomy and cardiovascular modules when teaching coronary circulation, sulci, and surface landmarks, and for clinical correlations in cardiology lectures on acute coronary syndrome and ECG localization.
