Description
Oriented in anterior anatomical position, the heart is rendered with the ascending aorta emerging superiorly and giving rise to the right and left coronary arteries from the aortic sinuses. The right coronary artery courses in the right atrioventricular (coronary) sulcus toward the acute margin, while the left main coronary artery passes between the pulmonary trunk and left auricle before dividing into the anterior interventricular (left anterior descending) artery in the anterior interventricular sulcus and the circumflex artery in the left atrioventricular groove. Superficial cardiac veins accompany the arterial pattern on the epicardial surface, with tributaries tracking the interventricular and atrioventricular grooves. Clear surface landmarks include the right ventricle forming most of the anterior face and the left ventricle tapering inferiorly toward the apex. Anterior coronary mapping matters because it corresponds directly to the ischemic territories implicated in acute coronary syndrome and to the ECG leads that sample them. Occlusion of the LAD along the anterior interventricular sulcus explains classic anterior wall infarction patterns and its proximity to the anterior papillary muscle and ventricular septum helps clinicians anticipate mechanical complications. Interventional planning also benefits from this perspective, since catheter angiography and coronary CT angiography reporting often describes lesions by segment along the RCA, left main, LAD, and circumflex as they wrap around the atrioventricular grooves.
