Description

Arising from the left coronary artery, the anterior interventricular artery (left anterior descending, LAD) descends in the anterior interventricular sulcus toward the cardiac apex, accompanied by the great cardiac vein in the same groove. From its course on the sternocostal surface, anterior ventricular branches spread laterally over the anterior wall of the left ventricle, while smaller right ventricular branches can cross the sulcus to supply the anterior right ventricle. Superiorly, the aortic root and ascending aorta sit posterior and slightly to the right of the pulmonary trunk, framing the proximal coronary origins near the left atrial appendage. For teaching coronary territories, this angle makes the LAD and its anterior ventricular branches easy to trace over the left ventricular myocardium, where they supply the anterior wall and much of the anterior interventricular septum via septal perforators (not the focus here, but implied by the parent vessel). Occlusion proximal to major anterior ventricular branches is the classic substrate for an anterior ST-elevation myocardial infarction, and the spatial relationship to the anterior interventricular sulcus helps learners map ECG changes to the infarct bed. A clean anterior surface view also supports discussions of coronary dominance and collateral flow without the clutter of posterior vessels. Cardiology and anatomy faculty can drop this rendering into lectures on coronary circulation, ischemic heart disease, or catheter-based revascularization, and it also fits figure needs for textbooks describing LAD lesions, bypass graft targets, or coronary CT angiography orientation.
