Centered in the pelvis, the uterus sits midline with the cervix inferiorly and the fundus superiorly, flanked bilaterally by the uterine (fallopian) tubes as they course laterally toward the ovaries. Scattered ectopic endometrial implants appear along the peritoneal surfaces adjacent to the uterine serosa and adnexa, with deeper infiltrating foci suggested where lesions cluster near the tubo-ovarian region. Small ovarian cysts consistent with early endometriomas occupy the ovarian cortex, and fibrous adhesions bridge normally separate planes between ovary, tube, and surrounding pelvic peritoneum. Adhesions distort anatomy. Stage 3 (moderate) endometriosis matters because pelvic pain and subfertility often track with tubo-ovarian adhesions rather than with superficial peritoneal implants alone, and this is the stage where altered tubal pickup and restricted ovarian mobility become clinically tangible. The distribution also mirrors common operative findings at laparoscopy: endometriotic implants on peritoneum with dense bands tethering the ovary posteriorly toward the ovarian fossa and, in some cases, toward the posterior leaf of the broad ligament and cul-de-sac, complicating dissection and increasing risk to the ureter and uterine vessels during adhesiolysis. In teaching, it clarifies why rASRM staging weighs adhesions and endometrioma size when correlating symptoms, infertility workup, and surgical planning.
