Description
Centered in the pelvis, the uterus sits midline with the fallopian tubes extending laterally toward the ovaries, which are shown with focal cystic change and surface irregularity consistent with small endometriomas. Scattered ectopic endometrial implants appear along the peritoneum and adnexa, with deeper plaques tracking posteriorly toward the cul-de-sac (pouch of Douglas) and uterosacral ligament region. Fibrotic adhesions tether the ovaries and tubes to adjacent peritoneal surfaces, narrowing the normal anatomic planes and distorting the relationship between fimbriae and ovarian cortex. Stage III (moderate) endometriosis matters because the disease is no longer limited to superficial peritoneal speckling; deep implants and adhesions begin to remodel pelvic anatomy in ways that directly affect symptoms and fertility. Restricted tubo-ovarian mobility impairs oocyte pickup and can contribute to subfertility even when tubal patency is preserved on hysterosalpingography. Pain correlates poorly with stage, but deep posterior lesions near the uterosacral ligaments and rectovaginal septum often align with deep dyspareunia and cyclic dyschezia. Distorted anatomy complicates surgery.
