Description
Centered in the pelvis, the uterus is shown with the bilateral fallopian tubes extending laterally toward the ovaries, creating a clear map of the adnexa and the peritoneal surfaces they contact. Dark, patchy implants consistent with ectopic endometrial tissue are distributed over the uterine serosa, along segments of the tubes, and on the ovarian surfaces, with the ovaries positioned posterolateral to the uterine fundus. The lesion pattern implies involvement of pelvic peritoneum as well as reproductive organs, typical of stage 2 disease where implants are more numerous and can extend beyond a purely superficial peppered appearance. Stage 2 endometriosis (often labeled mild in surgical staging, yet clinically capable of moderate symptom burden) matters because it is frequently the point at which patients develop persistent dysmenorrhea, deep dyspareunia, or subfertility without a large endometrioma or dense adhesive disease to “explain” symptoms on imaging. Laparoscopy remains the reference standard for confirming peritoneal implants and assigning stage, and this spread pattern helps teach why pain can localize to the posterior compartment even when the uterus and ovaries look relatively preserved. Small lesions can still drive neuroinflammation. The distribution also guides operative planning, since ablation or excision near the uterosacral region and adnexa demands attention to the ureter and ovarian reserve.
