Description
Centered in the pelvic cavity, the uterus is shown with ectopic endometrial implants scattered across the serosal surface, extending laterally toward the broad ligaments and adnexa. From the uterine cornua, the fallopian tubes course posterolaterally to the ovaries, where irregular dark foci mark deposits along the tubal serosa, ovarian cortex, and the peritoneum of the ovarian fossae. Mild stage 2 disease logically includes involvement of adjacent pelvic peritoneum, often along the posterior leaf of the broad ligament and toward the rectouterine pouch (pouch of Douglas), where deeper implants tend to cluster near the uterosacral ligaments. Small lesions, big symptoms. Stage 2 endometriosis corresponds to mild spread with more numerous implants and some deeper infiltration, a pattern that helps explain why pain severity can outstrip what is seen grossly at laparoscopy. Lesions on the uterosacral ligaments and posterior cul-de-sac correlate with deep dyspareunia and cyclical pelvic pain, while tubal and ovarian surface disease can contribute to subfertility through local inflammation, altered tubo-ovarian motility, and early adhesive change even when the tubes remain patent. For trainees, the distribution also reinforces that endometriosis is primarily a peritoneal disease rather than a problem confined to the uterine cavity.
