Description
Centered within the bony pelvis, the uterus sits superior to the vaginal canal, tapering inferiorly into the cervix while the uterine tubes course laterally toward the ovaries near the pelvic sidewalls. Along the inferior vestibule, the paired greater vestibular (Bartholin) glands occupy the superficial perineal pouch, positioned posterolateral to the vaginal orifice and deep to the posterior portions of the labia majora. Each gland’s duct runs anteromedially to open into the vestibule, classically at about the 4 and 8 o’clock positions relative to the introitus. Orientation is reinforced by the pelvic ring and pubic arch framing the anterior perineum. Locating the Bartholin gland precisely matters when evaluating a tender vulvar mass and planning drainage. Obstruction of the duct produces a Bartholin cyst, and secondary infection can form a Bartholin abscess; both typically present at the posterolateral vestibule rather than the midline. For procedural teaching, this relationship helps you avoid the midline perineal body and guides incision placement on the mucosal side of the vestibule during marsupialization, with attention to adjacent bulbospongiosus muscle and vestibular bulbs.
