Supraclavicular Nerve Associated With the Mammary Gland

Arising from the superficial cervical plexus, the supraclavicular nerve descends inferiorly over the clavicular region to reach the upper pole of the female mammary gland, where its cutaneous branches course within the superficial fascia anterior to pectoralis major. Lobules of glandular tissue and surrounding adipose are arranged around radially oriented lactiferous ducts converging toward the nipple and areola, while fibrous septa (Cooper ligaments) tether the skin to deeper fascia. Small vessels travel with the neural branches, with the neurovascular strands remaining superficial relative to the glandular parenchyma and deep fascia. Sensory territory concentrates in the superior and superomedial breast, near the clavicle. Mapping the supraclavicular contribution matters because breast sensation is not limited to intercostal nerves, and patients often report altered sensation or neuropathic pain in the upper breast after procedures that extend toward the infraclavicular or supraclavicular region. During mastectomy, lumpectomy, subpectoral implant placement, or port and pacemaker pocket creation, traction or transection of these superficial branches can produce dysesthesia along the clavicular chest wall and superior pole of the breast. This course also helps explain why a supraclavicular regional block can cover the upper breast skin, but will not anesthetize deeper glandular pain. Clear boundaries. Clear expectations.
