Shown from an anterior perspective, the female pelvis is opened to reveal the uterus with its fundus superiorly, the cervix leading inferiorly into the vaginal canal, and the paired uterine tubes arching laterally toward the ovaries. At the level of the vulvovaginal vestibule, the greater vestibular (Bartholin) glands sit posterolateral to the vaginal orifice, deep to the posterior parts of the labia majora and adjacent to the bulbs of the vestibule. Their ducts course anteromedially to open into the vestibule at about the 4 and 8 o’clock positions relative to the introitus. Small structures, big consequences. Bartholin gland anatomy matters because obstruction of the duct commonly produces a Bartholin cyst, and secondary infection can form a painful abscess that distorts the posterior vestibule and makes walking or intercourse difficult. This view helps clarify why drainage procedures target the ductal region at the posterolateral introitus, and why marsupialization or Word catheter placement aims to create a new, durable duct opening while avoiding injury to adjacent erectile tissue of the vestibular bulb. In patients over 40, a persistent unilateral mass raises concern for Bartholin gland carcinoma and changes the threshold for biopsy or excision. Use this illustration for gynecology and urogenital anatomy teaching (pelvis, perineum, and vulva), for patient-facing counseling materials explaining a Bartholin cyst or abscess, and for clinical reference in emergency medicine or outpatient procedures involving incision and drainage of the posterior vestibule.
