Description
Rendered in lateral profile on a male lower limb, the semimembranosus is highlighted along the posterior thigh, lying deep to the more superficial hamstring mass and running from the ischial tuberosity toward the medial side of the proximal tibia. Proximally it sits medial to the long head of biceps femoris and adjacent to semitendinosus, while distally its tendon passes posterior to the medial femoral condyle to reach the posteromedial tibial condyle. The femur forms the central skeletal landmark, with the patella shown anterior to the knee and the tibia and fibula extending inferiorly toward the ankle and tarsals. Orientation is clear. Semimembranosus matters because its distal tendon is a key landmark at the posteromedial knee, where expansions reinforce the posterior capsule and contribute to the oblique popliteal ligament, directly affecting knee stability in extension and during tibial rotation. Strains and proximal avulsions at the ischial tuberosity are common in sprinting and kicking sports, but clinicians often localize posteromedial knee pain to the semimembranosus tendon and its adjacent bursa, a frequent source of symptoms that can mimic meniscal pathology. The muscle also helps define the superomedial boundary of the popliteal fossa, where its relationship to the popliteal vessels and tibial nerve becomes relevant during posterior approaches and ultrasound-guided assessment.
