Description
Arising from the medulla and exiting the skull via the jugular foramen, the vagus nerve (cranial nerve X) descends in the upper neck as a trunk marked by its superior (jugular) and inferior (nodose) ganglia near the skull base. Branches to the pharynx and larynx course anteromedially toward the pharyngeal wall, while the main vagal trunk continues inferiorly within the carotid sheath, positioned between the internal jugular vein laterally and the internal or common carotid artery medially. Anteriorly, the tongue musculature is rendered in layered detail, and yellow cranial nerves traverse the floor of mouth toward the lingual complex, with lingual arterial supply shown inferior to the tongue. Teaching the vagus at the level of its ganglia clarifies two frequent points of confusion: the sensory ganglion distribution (somatic afferents in the superior ganglion, visceral afferents in the inferior ganglion) and why jugular foramen pathology produces mixed lower cranial neuropathies. This is the corridor implicated in vagal schwannoma, glomus jugulare lesions, and iatrogenic vagus injury during high carotid endarterectomy or skull base approaches; the close carotid sheath relationships explain associated hoarseness, dysphagia, and autonomic disturbance. Small branches matter here, the pharyngeal plexus, the superior laryngeal nerve, and the recurrent laryngeal pathway that ultimately governs vocal fold motion.
