Description
Arising from the medulla oblongata as cranial nerve X, the vagus nerve exits the skull via the jugular foramen, then descends within the carotid sheath between the internal jugular vein laterally and the internal carotid artery, then common carotid artery, medially. Superiorly, pharyngeal branches course anteromedially toward the pharyngeal plexus, while the superior laryngeal nerve divides into an internal branch piercing the thyrohyoid membrane and an external branch running inferomedially along the inferior pharyngeal constrictor toward the cricothyroid. Inferiorly, recurrent laryngeal branches loop back toward the larynx, ascending in the tracheoesophageal groove deep to the thyroid gland. Long parasympathetic trunks continue into the thorax and upper abdomen. Clinically, the cervical vagus is a key landmark during carotid endarterectomy and anterior cervical approaches, where retraction within the carotid sheath can contribute to postoperative dysphonia or dysphagia. Recurrent laryngeal nerve injury, classically on the left where it loops under the aortic arch, produces hoarseness from ipsilateral vocal fold paresis, and the illustration helps clarify why mediastinal pathology and thyroid surgery threaten this branch. Expect the superior laryngeal nerve external branch to be highlighted for its role in pitch control via the cricothyroid. Small branches, big consequences.
