Description
Posteriorly, the gallbladder lies inferior to the liver margin, its neck narrowing into the cystic duct as it courses medially to meet the common hepatic duct and form the common bile duct. Draped along the cystic duct and gallbladder neck, the cystic plexus appears as fine autonomic nerve filaments with small ganglia clustered near the hepatobiliary hilum, closely related to the bile duct wall. Proximally, these fibers track toward the hepatic plexus at the porta hepatis and the periarterial neural tissue around the hepatic artery, while distally they follow the extrahepatic bile duct toward the retroduodenal segment. That relationship matters in cholecystectomy and biliary reconstruction, where traction or diathermy near Calot’s triangle can disrupt autonomic input to the gallbladder and extrahepatic ducts and produce postoperative dysmotility or sphincter of Oddi spasm like pain. A posterior perspective clarifies why perineural dissection along the cystic duct can be bloody and uncomfortable for the patient, the plexus often running with small vessels and lymphatics and sitting immediately adjacent to the ductal adventitia.
