Description

Anterior thoracic anatomy is centered on the heart with the ascending aorta continuing into the aortic arch, the pulmonary trunk bifurcating into right and left pulmonary arteries, and the superior vena cava entering the right atrium on the patient’s right. Fine autonomic nerve strands and small ganglia of the cardiac plexus cluster around the inferior aspect of the aortic arch and extend inferiorly toward the base of the heart, with contributions tracking along the great vessels. Superiorly and posteriorly, the trachea and carina sit behind the arch, framing the region where the superficial and deep cardiac plexuses interconnect. Green lymphatic vessels and nodal groups in the superior mediastinum provide topographic context alongside the neurovascular structures. Clinical teaching often struggles with the cardiac plexus because its fibers are small but surgically relevant, and this anterior orientation makes their relationship to the aortic arch and pulmonary artery bifurcation easy to parse. Injury or traction in this compartment can contribute to postoperative dysautonomia after aortic arch work, pulmonary artery reconstruction, or mediastinal lymph node dissection, and it also explains why sympathetic fibers that descend from the cervical and upper thoracic chain can influence heart rate and contractility at the base of the heart.
