Description
Enlarged lobules of the mammary gland fill the central breast parenchyma, with clusters of secretory alveoli arranged in lobules that drain into progressively larger lactiferous ducts coursing anteriorly toward the nipple-areolar complex. Superficial adipose tissue sits anterolaterally and between glandular compartments, while fibrous septa (Cooper ligaments) extend from the dermis toward the deep fascia to tether skin to the underlying breast stroma. Posterior to the gland, the retromammary space separates the breast from pectoralis major, and the ribs form the deep osseous boundary of the anterior chest wall; small arteries, veins, and cutaneous nerves track through the stroma alongside the ductal tree. Pregnancy drives ductal proliferation and lobuloalveolar maturation under estrogen, progesterone, and prolactin, so this sectional anatomy is the clearest way to teach why the pregnant breast becomes denser and more tender while the proportion of adipose tissue decreases relative to glandular tissue. That shift matters in practice: increased parenchymal density can reduce mammographic sensitivity and changes the baseline appearance on ultrasound, and milk stasis within terminal ducts and lobules predisposes to plugged ducts and lactational mastitis in the early postpartum period. Orientation to the retromammary plane is also surgical, it is the natural dissection layer used in mastectomy and in subglandular implant placement.
