Anterior thoracic soft tissues are rendered with the mammary gland superimposed on the pectoralis major, its deep surface separated from the fascia by the retromammary space. Lobules of glandular tissue converge into lactiferous ducts that course anteromedially toward the nipple and areola, while adipose tissue fills the interlobular compartments and shapes the breast mound. Laterally, the breast tail (axillary tail of Spence) extends toward the axilla, where lymphatic vessels and nodes sit in close relationship to the lateral thoracic wall and the proximal arm. Superficial veins and accompanying cutaneous nerves are traced across the anterior chest. Lymphatic mapping is the point of this view. Most drainage follows a lateral route to the anterior (pectoral) axillary nodes, but a clinically meaningful medial pathway reaches the parasternal (internal thoracic) nodes, a route that explains contralateral spread and “hidden” nodal disease when the axilla is negative. The vascular supply, classically from perforators of the internal thoracic artery and contributions from the lateral thoracic and thoracoacromial systems, also frames common surgical planes for mastectomy and reconstructive flap planning while preserving perforators when indicated.
