Description

Anterior thoracic anatomy is presented with both mammary glands in situ, centered on the nipple and areola complex and extending radially through the glandular lobules and lactiferous ducts toward the axilla. Lobules occupy the superficial pectoral fascia and lie anterior to pectoralis major, while the retromammary space separates the gland from the underlying muscle and chest wall. Superficial vessels course within the subcutaneous tissue, and deeper perforators reach the gland from the internal thoracic and lateral thoracic territories. Lymphatic vessels track laterally toward the anterior (pectoral) axillary nodes and medially toward parasternal nodes, with sensory innervation following the anterolateral and anteromedial cutaneous branches of the intercostal nerves. Clinical decision making in breast disease often hinges on these exact pathways. Axillary drainage explains the first-echelon spread pattern in invasive carcinoma and underpins sentinel lymph node biopsy, while medial drainage to internal mammary nodes influences staging and radiotherapy fields. The relationship to pectoralis major and the retromammary plane matters in mastectomy and implant-based reconstruction, and the depiction of vascular and nerve courses supports safe incision planning around the inframammary fold and periareolar approaches.
