Description
Running across the lateral and posterior skull, the superficial temporal artery ascends anterior to the auricle while the occipital artery courses posteriorly toward the scalp, both set against clearly defined cranial sutures. Inferiorly, the common carotid artery ascends within the neck and bifurcates near the upper cervical region into the internal carotid artery, directed superiorly toward the cranial base, and the external carotid artery, which gives off branches to the face and scalp. Posterior to the carotid sheath region, the vertebral arteries track superiorly through the transverse foramina of the cervical vertebrae toward the foramen magnum. Bony landmarks remain dominant, including the occipital bone, mastoid region of the temporal bone, cervical vertebrae, and the superior thoracic cage with clavicle and scapular margin. A posterior oblique perspective like this helps correlate extracranial arterial pathways with palpable and procedural landmarks on the skull and upper neck, where small changes in course can matter. Scalp and facial lacerations often bleed briskly because branches of the external carotid artery are tethered in dense connective tissue, and this view clarifies why the superficial temporal and occipital territories can be hard to compress effectively. It also supports teaching the clinical logic of carotid endarterectomy and carotid bifurcation anatomy, and why vertebral artery injury is a concern with cervical trauma and posterior instrumentation.
