Description
Anterior and slightly oblique, the male lower limb is rendered with the cruris fascia (deep fascia of the leg) as a dense investing sheath enveloping the tibia and fibula and sending intermuscular septa toward the bones to partition the anterior, lateral, and posterior compartments. Proximally it continues from the fascia lata below the knee, while distally it thickens toward the ankle and blends with the retinacula that bind the long tendons as they cross the tibiotalar region. Extensor tendons course anteromedially toward the dorsum of the foot, the fibularis (peroneal) tendons track posterolateral to the lateral malleolus, and the Achilles tendon dominates posteriorly toward the calcaneus. Compartment anatomy is the point. A restrictive crural aponeurosis limits outward expansion of swollen muscle, which is why acute compartment syndrome can rapidly compromise the deep fibular nerve and anterior tibial vessels in the anterior compartment, or the posterior tibial neurovascular bundle in the deep posterior compartment, after tibial shaft fracture or reperfusion. This view also supports teaching of fasciotomy planning, including the standard two-incision technique that releases all four compartments by opening the fascia and intermuscular septa in safe longitudinal planes. Use it for lower-limb regional anatomy courses, surgical anatomy chapters on leg compartments and fasciotomy, sports medicine discussions of exertional compartment syndrome, or patient-facing materials explaining why a tight fascial sheath can produce pain and neurologic symptoms with swelling.
