Centered at the thoracoabdominal junction, the diaphragm appears as a bilaterally domed musculotendinous sheet with a broad peripheral muscular rim converging medially on the central tendon. Costal fibers arise from the inner surfaces of the lower ribs and costal cartilages, sternal slips attach near the xiphoid process, and crural fibers descend inferiorly from the lumbar vertebrae to form right and left crura flanking the aortic hiatus. Posteriorly, the diaphragm relates to the esophageal hiatus and caval opening, while its superior surface faces the pleura and pericardium and its inferior surface overlies the liver on the right and stomach and spleen on the left. Clear orientation. The phrenic nerves descend to the muscle, with motor supply reaching the diaphragm from C3 to C5. This view matters because diaphragmatic anatomy underpins how clinicians interpret an elevated hemidiaphragm on chest radiograph, paradoxical motion on fluoroscopic sniff testing, or unilateral diaphragmatic weakness after cardiothoracic surgery affecting the phrenic nerve. The domes and crura also define pathways for herniation, with the posterolateral region classically involved in Bochdalek hernias and the esophageal hiatus relevant to sliding hiatal hernia and reflux, while the close apposition to the pericardium frames referred shoulder pain via the phrenic nerve.
