Description
Seen from a posterior perspective through a semi-transparent back, the diaphragm forms a bilaterally domed partition between the lower thoracic cavity and upper abdomen. Muscle fibers radiate from the peripheral costal margin and xiphoid region toward the central tendon, while the lumbar part anchors inferiorly via the right and left crura to the anterolateral surfaces of the upper lumbar vertebral bodies and intervertebral discs. Medial arcuate ligaments bridge over psoas major and lateral arcuate ligaments bridge over quadratus lumborum, creating distinct posterior bands that frame the aortic hiatus posteriorly and the esophageal hiatus slightly anterior and to the left of midline. The course of the phrenic nerves is implied as they descend on the pericardium to reach the superior surface of the diaphragm. Posterior diaphragm anatomy matters when you are teaching or operating around the diaphragmatic hiatuses. Sliding hiatal hernia and reflux disease relate directly to the esophageal hiatus and the slinglike fibers of the right crus, and this view helps clarify why crural repair and fundoplication are oriented around posterior mediastinal landmarks rather than the anterior costal attachments. It also supports understanding referred shoulder pain from diaphragmatic pleura or peritoneum via C3 to C5 phrenic afferents. Small details, big consequences.
