Description
Coma-shaped epididymis contours along the posterior aspect of the testis, with its enlarged head (caput epididymidis) positioned superiorly at the superior pole, the body (corpus) descending posterolaterally, and the tail (cauda) turning inferiorly to become continuous with the ductus deferens. Fine efferent ductules enter the head from the rete testis, while the epididymal duct coils densely through the body toward the tail. Close apposition to the tunica albuginea and the posterior testicular vessels helps orient the viewer to the testicular hilum region. Spatial relationships matter here. Clinically, this anatomy underpins the typical posterior scrotal pain pattern of epididymitis, where inflammation begins in the tail and tracks proximally along the epididymal duct, sometimes involving the testis as epididymo-orchitis. Distinguishing the epididymis from the testis is also central when teaching torsion: a torsed spermatic cord compromises testicular perfusion, while isolated epididymal pathology more often preserves intratesticular flow on Doppler ultrasound. Surgeons also rely on the head and tail landmarks during vasectomy counseling and during epididymal sperm aspiration, since the cauda contains the highest concentration of mature sperm.
