Description
Cross-sectioned craniofacial bones frame the orbit around a centrally placed globe, with the superior and inferior ophthalmic veins traced in blue along the superior and inferior aspects of the orbital contents. Medially, venous channels course toward the ethmoidal region and the superior orbital fissure, while laterally smaller venules skirt the lacrimal gland region and converge posteriorly toward the orbital apex. Posterior to the sclera, the optic nerve exits the globe, and red arterial counterparts, consistent with the ophthalmic artery and its branches, run alongside the venous pathways. Fine vortex venules are suggested at the posterior globe, draining the choroid toward the ophthalmic venous system. Venous anatomy of the orbit matters because these channels connect directly, and often valvelessly, with the cavernous sinus via the superior orbital fissure, a pathway implicated in cavernous sinus thrombosis arising from facial or sinonasal infection. Surgical corridors such as medial orbital decompression for thyroid eye disease or endoscopic approaches along the lamina papyracea place the ethmoidal veins and the superior ophthalmic vein at risk for hemorrhage and postoperative orbital congestion. Small structures, big consequences.
