Framed within a skull cross section through the orbit, the voluminous superior ophthalmic vein courses along the superior aspect of the orbital cavity, superficial to the extraocular muscle cone and in close relationship to the orbital roof. Medially, its tributaries align with the angular and supratrochlear venous pathways near the superomedial orbit, while laterally it collects venules from the superior rectus and lacrimal region. Anteriorly the vessel network fans across the episclera and conjunctival surface, then converges posteriorly toward the orbital apex where the vein tracks toward the superior orbital fissure. Venous drainage of the orbit is clinically unforgiving because the superior ophthalmic vein provides a direct conduit between facial venous channels and the cavernous sinus, and this view makes that connection anatomically intuitive rather than theoretical. Think cavernous sinus thrombosis from a midface infection, or carotid cavernous fistula where the superior ophthalmic vein enlarges and becomes a key sign on CT or MR venography. Big vessel.
