Entering the orbit at its anterosuperior quadrant, the superior ophthalmic vein courses posteriorly along the roof of the orbital cavity, collecting tributaries from the globe and adjacent extraocular tissues before tracking toward the orbital apex. Medial to the lateral rectus region and superior to the optic nerve, it runs in close company with the ophthalmic artery and small venules draining the eyelids and conjunctiva. A section through the skull base and orbit also brings the bony margins of the orbit into context, with the nasal cavity positioned medially and inferiorly relative to the orbital contents. Spatial relationships are the point. Venous drainage of the orbit matters because it is one of the clearest anatomic routes by which infection and thrombosis can move between superficial facial territories and the cavernous sinus. The superior ophthalmic vein is a frequent conduit in cavernous sinus thrombosis secondary to facial or sinus infection, and its dilation on CT or MRI can act as a practical clue to carotid cavernous fistula or elevated orbital venous pressure. Surgical and interventional planning also benefits from this view, since the vein’s position near the optic nerve and orbital apex frames the risk envelope for orbital decompression, endoscopic approaches to the medial orbit, and transvenous access routes.
