Description
Arising at the medial canthus from angular and supraorbital tributaries, the superior ophthalmic vein courses posteriorly along the superior orbit, closely related to the levator palpebrae superioris and superior rectus complex. It then tracks toward the orbital apex, lying superior to the optic nerve as both approach the optic canal and superior orbital fissure region. A skull section framing the orbits would also place the maxilla and upper dentition inferiorly, with the internal cranial cavity superior and posterior to the orbital roofs. Venules and companion arterial channels encircle the globe, while the optic nerve runs posteromedially toward the cranial compartment. Venous anatomy here is not academic trivia. The superior ophthalmic vein is a principal route between the facial venous system and the cavernous sinus, so infections from the “danger triangle” of the face can spread posteriorly, producing cavernous sinus thrombosis with painful ophthalmoplegia and chemosis. For orbital surgeons, its position at the apex matters when decompressing the orbit or operating near the superior orbital fissure, where congestion or injury can complicate exposure and increase hemorrhage. Small variations in its caliber and tributary pattern also influence the appearance of orbital venous congestion on angiography and contrast-enhanced CT or MRI. Use this illustration in head and neck anatomy curricula when teaching orbital spaces and venous communications, and in clinical neuroanatomy modules that connect cranial nerves III, IV, V1, and VI to cavernous sinus disease.
