Viewed from superior into the cranial cavity, the internal surface of the anterior, middle, and posterior cranial fossae frames the central sella turcica, with the hypophyseal arterial branches arising from the intracavernous or supraclinoid internal carotid artery and coursing medially toward the hypophysis. Anterior (superior) hypophyseal arteries typically sweep toward the optic chiasm and pituitary stalk (infundibulum) at the diaphragma sellae, while inferior hypophyseal arteries approach the posterior lobe from the cavernous segment, running close to the body of the sphenoid. Around this midline corridor, the Circle of Willis is established by the anterior cerebral arteries and anterior communicating artery anteriorly, and the posterior communicating arteries linking to the posterior cerebral arteries posteriorly; vertebral arteries unite as the basilar artery along the clivus. Foraminal landmarks of the skull base sit lateral to the sella, including the carotid canal region and foramina of the sphenoid. That relationship between hypophyseal vessels and the sella turcica matters in transsphenoidal pituitary surgery, where the carotid protuberances and cavernous sinus define the lateral limits and where injury to hypophyseal branches can compromise pituitary perfusion. Even small-caliber arterioles become clinically visible in outcomes, since disruption of the superior hypophyseal system affects the portal circulation to the anterior pituitary and can contribute to postoperative hypopituitarism, while cavernous carotid pathology can present with sellar or parasellar symptoms.
