Description
Brainstem nuclei occupy the core of the medulla oblongata, pons, and midbrain, arranged in longitudinal columns that relate closely to the floor of the fourth ventricle and the cerebral aqueduct. Medial territories emphasize motor groups, including the hypoglossal nucleus, abducens nucleus, and oculomotor and trochlear nuclei, while more lateral regions accommodate sensory nuclei such as the spinal trigeminal nucleus, solitary nucleus (nucleus tractus solitarii), and the vestibular and cochlear nuclei. Between these columns sit integrative and autonomic centers, with the reticular formation spanning much of the tegmentum and the inferior and superior colliculi forming prominent dorsal midbrain landmarks. The organization is compact. Clinical localization often hinges on the relationship between a cranial nerve nucleus and adjacent long tracts, and this illustration supports that bedside logic by placing nuclei within their medullary, pontine, or midbrain levels rather than as an abstract list. Internuclear connections are implied by proximity, for example the medial longitudinal fasciculus near ocular motor nuclei in the dorsal pons and midbrain, a common substrate for internuclear ophthalmoplegia in multiple sclerosis. Vascular syndromes become easier to teach when the viewer can separate medial paramedian territories (often affected by basilar perforator infarcts) from lateral tegmental regions where Wallenberg (lateral medullary) patterns involve the nucleus ambiguus, vestibular nuclei, and spinal trigeminal nucleus.
