Description
Oriented as an inferior view into the cranial cavity, the skull base is opened to reveal the vertebrobasilar system and its pontine arteries running on the ventral surface of the pons. The basilar artery ascends in the midline on the clivus, giving off multiple short circumferential and paramedian branches that penetrate the pontine substance, while the vertebral arteries approach inferiorly near the foramen magnum. More rostrally, the basilar bifurcation leads into the posterior cerebral arteries, and the posterior communicating arteries connect toward the internal carotid circulation of the Circle of Willis. Pontine perforators matter because they are end-arteries with limited collateral flow, so small-vessel occlusion can produce focal brainstem syndromes that correlate tightly with tract and cranial nerve nucleus anatomy. This inferior cranial base perspective also reinforces surgical and endovascular landmarks, including the relationship of the basilar trunk to the clivus and dorsum sellae, and why perforator preservation becomes the limiting factor in basilar aneurysm treatment or angioplasty and stenting.
