Description
Arising from the proximal posterior cerebral artery near its origin at the basilar apex, the posteromedial central arteries course superiorly and medially to enter the posterior perforated substance, sending fine perforators toward the thalamus and rostral midbrain. Their trajectories sit posterior to the optic chiasm and infundibulum and medial to the cerebral peduncles, in close relationship to the interpeduncular cistern. Surrounding skull base landmarks such as the dorsum sellae, clivus, and adjacent foramina provide bony orientation for the intracranial vascular layout. Small vessels. Big consequences. These perforating branches matter because they represent end-arterial supply to deep gray matter and brainstem structures where collateral flow is limited, so occlusion can produce focal thalamic infarction or paramedian midbrain syndromes. Variations in the perforator pattern, including contributions from the P1 segment and the presence of a fetal-type posterior cerebral artery, influence both ischemic risk and what you expect to see on angiography. For neurosurgical corridors around the basilar tip and posterior communicating region, the posteromedial perforators are the vessels you avoid at all costs during aneurysm clipping or endovascular manipulation.
