Description
Visible within a superior cranial cutaway, the circle of Willis gives rise to small posteromedial central arteries coursing posteriorly and medially from the posterior cerebral artery near the basilar bifurcation and posterior communicating artery junction. These perforators pass through the posterior perforated substance to enter the diencephalon, where they supply the paramedian thalamus and adjacent hypothalamus. Around them, you would expect to see the basilar artery on the ventral brainstem, paired posterior cerebral arteries curving around the midbrain, and the proximal segments of the internal carotid system more anteriorly. Calvarial bone and the cranial base frame the vascular field. Paramedian thalamic infarction is the clinical reason these vessels matter. Occlusion of a posteromedial perforator, or a single dominant trunk such as the artery of Percheron variant, can produce bilateral medial thalamic ischemia with altered mental status, vertical gaze palsy, and memory impairment, a pattern that can be missed if you only think in large territorial strokes. For endovascular and microsurgical planning, the posteromedial perforators also define a no-go zone around basilar tip aneurysms and posterior cerebral artery P1 segment interventions.
