Description
Oriented from a superior view into the cranial cavity, the basilar artery is centered on the ventral surface of the pons, formed by the union of the right and left vertebral arteries along the midline of the clivus. Multiple pontine arteries arise segmentally from the basilar trunk and sweep laterally across the pontine surface, with shorter paramedian perforators directed posteriorly into the pontine parenchyma and longer circumferential branches coursing toward the cerebellopontine angle. Posteriorly and superiorly, the basilar apex and proximal posterior cerebral arteries can be inferred, with the posterior communicating arteries contributing to the Circle of Willis. For teaching brainstem vascular territories, this angle clarifies why basilar trunk disease produces stereotyped deficits. Occlusion of paramedian pontine perforators can injure corticospinal tracts and medial lemniscus, while circumferential branch compromise can affect the lateral pons with facial nucleus involvement or trigeminal sensory deficits, and basilar thrombosis remains the feared substrate for locked-in syndrome.
