Description
Arising from the posterior aspect of the external carotid artery, the occipital artery ascends deep to the posterior belly of the digastric and courses posteriorly toward the mastoid region, then continues superiorly along the occiput. In lateral head and neck view, its stem is seen passing medial to the angle of the mandible and deep to the sternocleidomastoid before emerging near the superior nuchal line, where it divides into terminal scalp branches. Temporal muscle occupies the lateral calvaria anteriorly, while the auricle and adjacent bony landmarks help orient the vessel’s path relative to the mastoid process and occipital bone. Red arterial branches and blue companion veins form a dense superficial network over the scalp and posterior auricular region. Understanding the occipital artery’s relationship to the sternocleidomastoid and posterior scalp planes matters when planning posterior cervical or occipital incisions, where the vessel can be a source of brisk bleeding if not anticipated. Surgeons also use its caliber and predictable trajectory in reconstructive work, including occipital artery based scalp flaps, and interventionalists may target branches as feeders in scalp arteriovenous malformations. A key landmark. The artery’s anastomoses with the superficial temporal and posterior auricular arteries explain why scalp wounds can bleed profusely and why perfusion often remains preserved despite segmental injury.
