Description
Running as short, segmental slips within the transversospinalis group, the rotatores span from the transverse process of a vertebra to the lamina and spinous process of the vertebra one or two levels superior (rotatores breves and longi), most conspicuous in the thoracic region. Posteriorly, the muscle fibers sit deep to multifidus and medial to the longissimus thoracis, hugging the vertebral arches from the lower cervical spine through the upper lumbar levels. Ribs articulate laterally at the costovertebral and costotransverse joints while the scapulae and proximal humeri frame the shoulder girdle superficial to the thoracic cage. Small muscles, precise geography. Teaching spinal mechanics often stalls at “extension and rotation” unless learners can see the rotatores as short levers that link transverse processes to spinous processes, tightening across the posterior elements as the thoracic spine rotates. Their dense distribution over the mid thoracic vertebrae also explains why thoracic facet joint irritation and paraspinal strain can generate sharply localized, segmental tenderness near the spinous processes, a pattern clinicians frequently map during palpation and manual therapy. For posterior approaches to the thoracic spine, appreciating their depth relative to multifidus helps clarify what gets split or retracted before laminar exposure.
