Description
Entering the oral cavity through an open mandible, the tongue musculature is exposed around the median fibrous septum linguae, a vertical partition running in the sagittal plane from the dorsum toward the ventral surface. Intrinsic tongue fibers lie on either side of this midline septum, while the paired genioglossus muscles occupy the anterior body of the tongue and approach the septum medially from their mandibular origins at the genial tubercles. Superior and inferior relationships are clarified by the hard palate and maxillary dentition above, with the mandibular teeth and body of the mandible forming the inferior and anterior bony frame. Midline anatomy in the tongue matters because it dictates how disease and surgical planes behave. The septum linguae limits cross-midline spread of some tumors and infections within the intrinsic musculature, so depth of invasion assessment in oral tongue squamous cell carcinoma often hinges on understanding what is truly midline versus paramedian. It also guides surgical dissection during partial glossectomy and transoral approaches, where staying just lateral to the septum can preserve contralateral neurovascular structures and helps orient the course of the hypoglossal nerve and lingual artery in the floor of mouth region.
