Description
Highlighted in blue, the sphenoid bone occupies the central cranial base and appears laterally as the greater wing between the frontal bone superiorly, the temporal bone posteriorly, and the zygomatic bone anterolaterally at the orbit and temple. Its contribution to the lateral orbital wall sits posterior to the zygomatic bone and lateral to the ethmoid, while the pterygoid processes descend inferiorly, tucked medial to the mandibular ramus and posterior to the maxilla. Surrounding context includes the parietal squama and coronal suture superiorly, the zygomatic arch spanning anterior to the temporal fossa, and the maxilla and mandible with dentition in the facial skeleton. Inferiorly, the occipital region transitions into the upper cervical spine, with the atlas and axis aligned beneath the cranial base. A lateral skull view is where the sphenoid stops being an abstract wedge and becomes a surgical and trauma landmark, because the greater wing and adjacent foramina form the bony corridor for the middle meningeal artery and branches of the trigeminal nerve as they exit the cranial cavity. Thin bone here. Fractures across the sphenoid and orbital apex can threaten the optic canal and superior orbital fissure region, producing optic neuropathy or ophthalmoplegia, and the same neighborhood matters in transsphenoidal and endoscopic skull base routes aimed at the sellar region and cavernous sinus.
