Description
Seen in transverse section at the L1 vertebral level, the spinal cord parenchyma surrounds a small central canal and organizes into an H-shaped gray matter core with anterior (ventral) and posterior (dorsal) horns separated by the gray commissure. White matter forms the peripheral rim, where the posterior funiculus lies dorsally and the anterior and lateral funiculi occupy the ventral and lateral aspects, already reduced in bulk compared with thoracic levels as the cord tapers toward the conus medullaris. Ventral horn gray matter sits anteriorly, closer to the anterior median fissure, while the dorsal horns extend posteriorly toward the posterior median sulcus. Fine surface micro-texture emphasizes the pia-adherent contour of the cord. At L1 you are at a teaching sweet spot: the cord proper transitions into the conus medullaris, and the surrounding canal begins to accommodate the descending lumbar and sacral roots of the cauda equina (not shown in this isolated cord cross-section, but clinically inseparable from the level). This relationship underpins why lumbar puncture targets the L3 to L4 or L4 to L5 interspace to avoid direct spinal cord injury, while still accessing cerebrospinal fluid via the dural sac. It also frames classic syndromes in a level-specific way, for example, anterior spinal artery infarction preferentially affecting anterior horn cells and anterolateral white matter tracts, while sparing portions of the posterior columns.
