Centered in the thoracic spinal cord at the T2 level, the grey matter forms a compact H shape with slender anterior (ventral) horns and elongated posterior (dorsal) horns, joined by the grey commissure around the central canal. White matter surrounds this core as distinct funiculi, with an anterior funiculus ventral to the anterior median fissure, paired lateral funiculi, and a posterior funiculus separated by the posterior median sulcus. Thoracic patterning is apparent. The intermediolateral cell column (lateral horn) is expected between the dorsal and ventral horns at this level, positioned laterally and associated with preganglionic sympathetic neurons. Dorsal root entry and ventral root exit zones lie posterolaterally and anterolaterally along the cord surface, respectively, where spinal nerves form. T2 is a teaching sweet spot for distinguishing regional spinal cord anatomy: small ventral horns reflect fewer lower motor neuron pools than cervical or lumbosacral enlargements, while white matter organization emphasizes long tracts. Clinically, lesions near this segment can produce a sensory level over the upper thorax and influence sympathetic outflow to the head and upper limb via ascending pathways, a pattern that helps localize myelopathy. The lateral horn matters in cord syndromes and in surgical anatomy when correlating thoracic spinal cord injury with autonomic findings. Use this cross-section in neuroanatomy and gross anatomy curricula to anchor discussions of grey versus white matter, ascending and descending tracts, and segmental localization around the upper thoracic spine.
