Description

Centered on the sacral spinal cord at S3, the cross-section separates a butterfly-shaped substantia grisea from the surrounding substantia alba. Posterior (dorsal) gray horns appear slender, while the anterior (ventral) horns remain small but distinct in this caudal segment, framing the commissura grisea around the canalis centralis. A thin rim of white matter persists circumferentially, reduced compared with lumbar levels, reflecting the tapering of ascending and descending tracts as the cord approaches the conus medullaris. Orientation can be read by the deeper anterior median fissure relative to the shallower posterior median sulcus. S3 matters because it anchors the neuroanatomy of pelvic floor and sphincter control: the sacral parasympathetic outflow arises from S2 to S4, and motor neurons here contribute via the pudendal nerve to the external urethral and anal sphincters. This segment-level view helps correlate selective sacral deficits, for example saddle anesthesia and early bladder or bowel dysfunction in conus medullaris syndrome, versus a cauda equina lesion where peripheral roots are primarily affected. It also supports teaching the difference between gray matter injury patterns and long-tract signs when white matter is relatively sparse.
