Seen in lateral profile beneath semi-transparent skin, the thyrohyoideus spans from the oblique line of the thyroid cartilage superiorly to the inferior margin of the hyoid bone, sitting deep to the infrahyoid strap layer and anterior to the cervical vertebral column. Superior to it lie the suprahyoid structures around the mandible, while inferiorly the laryngeal framework continues toward the upper trachea and the manubrial level suggested by the clavicles and first ribs. Posterior landmarks include the cervical vertebrae; anteriorly the laryngeal prominence region is implied by the thyroid cartilage. Short, direct, and easy to orient. Because the thyrohyoideus elevates the larynx or depresses the hyoid depending on fixation, this view is a practical way to teach the mechanics of swallowing and phonation in relation to the hyoid bone and thyroid cartilage. Surgeons use the strap muscles, including thyrohyoideus, as a midline corridor in open tracheostomy and laryngeal exposure, and recognizing its attachment pattern helps clarify why superior laryngeal excursion is reduced after anterior neck scarring or postoperative strap muscle tethering. The close relationship to the laryngeal cartilages also makes it a useful landmark when discussing external palpation for airway assessment.
