Thoracic vertebral pedicle spans from the posterolateral aspect of the vertebral body to the neural arch, forming the short, thick bony bridge that frames the lateral wall of the vertebral canal. Its superior and inferior borders are contoured by the vertebral notches, which contribute to the intervertebral foramen when aligned with the adjacent vertebra. Posterior to the pedicle, the laminae converge toward the spinous process, while laterally the pedicle continues toward the transverse process and the superior and inferior articular processes with their zygapophyseal joint surfaces. Cortical bone is emphasized along the pedicle’s outer shell, with cancellous bone suggested internally. No soft tissue distracts. Pedicle anatomy matters because it defines the safest bony corridor for thoracic pedicle screw placement and it determines the boundary between the spinal canal medially and the exiting thoracic spinal nerve in the intervertebral foramen laterally. Narrow pedicle diameter and variable angulation in the mid thoracic levels increase the risk of medial breach with cord injury or lateral breach with radicular pain, so appreciating the pedicle’s relationship to the vertebral body and posterior elements is nonnegotiable in preoperative planning.
