Description

Emerging at the anterior floor of the mouth, the anterior lingual gland (glandula lingualis anterior, glands of Blandin and Nuhn) sits within the ventral musculature of the tongue just posterior to the mandibular symphysis. From an inferior perspective, its paired lobules lie lateral to the lingual frenulum and deep to the mucosa of the ventral tongue, with the genioglossus forming a medial muscular bulk and the geniohyoid and mylohyoid positioned inferiorly as the tongue meets the sublingual space. The gland’s small ducts open near the midline on the ventral surface close to the apex linguae. Small but specific. This angle matters because lesions of the ventral tongue often originate or spread along this glandular tissue plane, and surgeons need to distinguish a Blandin-Nuhn mucous retention cyst from pathology of the sublingual gland or minor salivary glands of the labial mucosa. The inferior view also clarifies why midline approaches for frenotomy or ventral tongue biopsies must respect adjacent vascular and ductal structures, including the deep lingual veins and the submandibular duct as it courses anteriorly in the floor of mouth. It is a practical anatomy problem, not a trivia point.
