Description
Dominating the anterolateral shoulder, the deltoid forms a triangular muscle mass superficial to the glenohumeral joint, with its broad origin spanning the lateral third of the clavicle anteriorly and the acromion and spine of the scapula superiorly and posteriorly. Fibers converge inferiorly to a narrow insertion on the deltoid tuberosity of the humerus, lying lateral to the bicipital groove and the tendon of the long head of biceps as it enters the intertubercular sulcus. A semi-transparent thoracic cage sits medial and inferior to the shoulder girdle, while the clavicle runs horizontally from the sternum toward the acromion, defining the superior border of the anterior shoulder. Striation direction is readable across the anterior, middle, and posterior fiber regions. Clean landmarks. Clinically, an anterior skin-level view of the deltoid clarifies why shoulder abduction is not a single-muscle action: the middle deltoid supplies the main abduction torque after initiation by supraspinatus, while the anterior fibers contribute flexion and internal rotation and can dominate when the scapula is protracted. That relationship matters when assessing axillary nerve injury after surgical neck fractures or anterior shoulder dislocation, where deltoid weakness and the sensory deficit over the lateral shoulder (superior lateral cutaneous nerve of arm) are key findings. The deltoid’s surface anatomy also guides safe intramuscular injection in the mid-deltoid region, avoiding the axillary nerve and posterior circumflex humeral vessels deep to the muscle.
