Description
Prominent on the anterolateral shoulder, the deltoid muscle forms a triangular cap over the glenohumeral joint, overlaid here on an anterior view of the male skeleton. Its clavicular (anterior) fibers arise from the lateral third of the clavicle, while the acromial portion spans from the acromion of the scapula, both converging distally toward the deltoid tuberosity on the lateral humeral shaft. Deep and medial to the muscle belly sit the humeral head and glenoid fossa, with the clavicle running horizontally superior to the rib cage and the scapula positioned posterolateral but referenced through its acromial contour. Bony landmarks stay readable. That matters. Anterior emphasis on the deltoid is useful because it separates what is palpable from what is actually moving at the shoulder: the deltoid’s anterior fibers flex and medially rotate the arm, while its lateral fibers drive abduction after the supraspinatus initiates the first degrees. Clinically, the relationship of the deltoid to the surgical neck of the humerus and quadrangular space explains why anterior shoulder dislocation or proximal humerus fracture can compromise the axillary nerve, producing deltoid weakness and loss of the lateral shoulder sensation. Surface anatomy also counts, since intramuscular injection sites rely on locating the acromion and staying in the mid-deltoid to reduce risk to the axillary nerve and posterior circumflex humeral artery.
