Description

Centered in a superior view of C2 (axis), the dens (odontoid process) rises from the vertebral body and narrows toward its apex, positioned anterior to the vertebral foramen and just medial to the paired superior articular facets. Lateral to the base of the dens, the pars interarticularis forms the bony isthmus between the superior and inferior articular facets, continuous with the posterior elements and leading into the laminae and spinous process. The animation subtly rotates and recenters the axis to keep the apex of the dens in profile while the surrounding articular surfaces come in and out of alignment. Orientation is kept anatomical, with anterior toward the dens and posterior toward the lamina and spinous process. Attention to the dens apex matters because it is the pivot for atlanto-axial rotation and a key landmark when discussing odontoid fractures, including Anderson and D'Alonzo Type I injuries at the tip and Type II injuries at the base with their well-known risk of nonunion. The sequential motion clarifies how the dens relates to the superior articular facets that articulate with the atlas and how the pars interarticularis bridges facet to facet, a region implicated in traumatic spondylolysis of C2 (hangman’s fracture) involving the posterior elements. Seeing these relationships shift with rotation helps explain why certain fracture lines destabilize the C1-C2 complex while others spare the atlanto-occipital unit. Small distances matter. Use this animation in gross anatomy and neuroanatomy teaching on the upper cervical spine, in radiology education when correlating a superior bony view with axial CT through C2, or in spine surgery lectures introducing posterior C2 screw trajectories and the surrounding facet anatomy. Anatomical accuracy verified by SciePro's Medical Advisory Board.
