Description
Pars interarticularis of the axis (C2) forms the stout bony bridge between the superior and inferior articular processes, positioned posterolateral to the vertebral body and lateral to the lamina. The superior articular facets sit cranial to this bridge and face superolaterally toward the atlas, while the inferior facets lie caudal and align with C3, framing the pars as a short segment of the neural arch near the pedicle-lamina junction. Surrounding landmarks that typically remain visible include the spinous process posteriorly and the vertebral foramen medially, with the dens located anteriorly on the body of C2 as the defining midline feature of the axis. Orientation matters here. Clinically, the C2 pars interarticularis is the classic site implicated in traumatic spondylolysis of the axis, the so-called hangman’s fracture, where bilateral defects can allow anterior translation of the C2 body on C3 and threaten spinal canal alignment. This fixed perspective helps you distinguish a true pars defect from fractures of the lamina or pedicle by isolating the articular pillar and showing how the superior and inferior facets are connected by a narrow isthmus of bone. The relationship to the facet joints is the teaching point.
